Alfacalcidol is converted to its active form, calcitriol, in the liver. Once converted, alfacalcidol acts similarly to natural vitamin D by promoting the absorption of calcium and phosphate in the intestines. Specifically, alfacalcidol enhances the absorption of these minerals from the digestive tract, which is crucial for maintaining healthy bone density and mineralization. It also regulates calcium levels in the blood by increasing the mobilization of calcium from bones and reducing its excretion through the kidneys. By ensuring adequate levels of calcium and phosphate in the blood, alfacalcidol helps to support bone health and prevent conditions like rickets and osteomalacia.

Alfacalcidol dosage recommendations are:

• General Use (excluding osteoporosis) - Adjust the dose to avoid hypercalcemia. Initial plasma levels should be measured weekly. The daily dose may be increased by increments of 0.5 mcg. Once stabilized, measure plasma levels every 2-4 weeks. Most adults respond to 1-3 mcg/day. Dose generally decreases with evidence of bone healing or normal plasma calcium levels. Maintenance dose ranges from 0.25 to 1 mcg/day. If hypercalcemia occurs, stop ALFAGET until plasma calcium returns to normal (about a week), then restart at half the previous dose.
• Osteoporosis - Usual dose is 0.5 mcg/day. Maintenance dose ranges from 0.5 to 1 mcg/day, not to be exceeded. For children with osteoporosis, the general dose ranges from 0.01 to 0.03 mcg/kg once a day.
• Renal Bone Disease (Renal Osteodystrophy) - For children weighing 20 kg and over, the dose is 0.04-0.08 mcg/kg/day. Children may require higher doses than adults and may need the adult dose.
• Hypoparathyroidism/Hyperparathyroidism - For adults, 1-4 mcg is given orally once a day. Severe hypocalcemia may be corrected more rapidly with higher doses of 3-5 mcg, along with calcium supplements. ALFAGET can be used for 2-3 weeks as preoperative treatment for primary or tertiary hyperparathyroidism surgery.

In the event of overdose, alfacalcidol should immediately be stopped and in severe hypercalcemia additional treatment with loop diuretics and intravenous fluids, or corticosteroids is done.

Take the missed dose as soon as you remember. If it’s almost time for the next dose, skip the missed dose. Do not double up.

Take tablet daily, preferably at the same time each day. Follow the dosage instructions provided by your healthcare provider.

Do not use Bone lift:

• If you have severe hypercalcemia.
• If you are allergic to any of the ingredients.

• Your doctor may prescribe a phosphate-binding agent along with One-Alpha to maintain the correct amount of phosphate in your blood.

Drug interactions with alfacalcidol include:

• Calcium supplements (increases risk of hypercalcemia)
• Thiazide diuretics (increases risk of hypercalcemia)
• Magnesium supplements (increases risk of hypercalcemia)
• Digitalis glycosides (increases risk of arrhythmias)
• Anticonvulsants (may alter vitamin D metabolism)
• Corticosteroids (may reduce the effectiveness of alfacalcidol)
• Phenytoin (may decrease alfacalcidol levels)
• Rifampin (may decrease alfacalcidol levels)
• Barbiturates (may decrease alfacalcidol levels)
• Estradiol (may increase alfacalcidol levels)
• Cholestyramine (may decrease alfacalcidol absorption)

As of now, there are no known interactions between this drug and food.
