    Aminophylline is a combination of theophylline (a methylxanthine) and ethylenediamine. Its mechanism involves inhibiting phosphodiesterase enzymes, which increases levels of cyclic adenosine monophosphate (cAMP) within cells. Elevated cAMP levels lead to relaxation of bronchial smooth muscle and reduced inflammation, which helps ease breathing. Ammonium chloride increases the acidity of urine and the respiratory tract secretions. By acidifying these secretions, ammonium chloride helps to break down mucus, making it easier to expel from the airways. This action promotes the clearing of mucus and helps relieve cough and congestion. Diphenhydramine is an antihistamine that works by blocking the H1 histamine receptors in the body. Histamine is a chemical released during allergic reactions, which binds to these receptors and causes symptoms such as itching, swelling, and vasodilation. By antagonizing the H1 receptors, diphenhydramine prevents histamine from binding and activating these receptors. This action helps to alleviate allergy symptoms, such as runny nose, sneezing, and itching. Additionally, diphenhydramine has anticholinergic properties, which contribute to its sedative effects and can also provide relief from nausea and motion sickness.

    Cosome-E 120ml Cough Syrup Dosage Recommendations are:

    • Cough and Asthma - Adults 10-20 ml taken every 4-6 hours as needed. Children (6-12 years) 5-10 ml every 4-6 hours as needed. Children (2-6 years) 2.5-5 ml every 4-6 hours as needed
    • Bronchitis - Adults 10-20 ml taken every 4-6 hours as needed. Children (6-12 years) 5-10 ml every 4-6 hours as needed. Children (2-6 years) 2.5-5 ml every 4-6 hours as needed
    • Allergic Rhinitis - Adults 10-20 ml taken every 4-6 hours as needed. Children (6-12 years) 5-10 ml every 4-6 hours as needed. Children (2-6 years) 2.5-5 ml every 4-6 hours as needed
  • In case of Overdose

    Seek immediate medical help. Symptoms of overdose may include nausea, vomiting, or gastrointestinal upset. Follow medical advice.

  • Missed Dose

    Take the missed dose as soon as you remember. If it’s almost time for the next dose, skip the missed dose. Do not double up.

  • How To Use

    Shake well before use. Measure the dose with a marked spoon or cup. Take as directed by your healthcare provider.

  • When Not to Use

    Aminophylline + ammonium chloride + diphenhydramine combination should not be used in the following situations:

    • Use Cosome E with caution in conditions such as angle-closure glaucoma, urinary retention, prostatic hyperplasia, myasthenia gravis, epilepsy or seizure disorders, asthma, bronchitis, chronic obstructive pulmonary disease (COPD), moderate to severe hepatic impairment, moderate to severe renal impairment, hypothyroidism, sepsis with multiorgan failure, shock, cessation of smoking, severe cardiac disease, coronary artery disease, congestive heart failure, hypertension, a history of peptic ulcer, or thyrotoxicosis.
    • Patients should be cautious when operating motor vehicles or machinery as Cosome E can cause sedation and dizziness.
    • Tolerance to the medication may develop with continuous use. Seek medical advice if insomnia persists, as it may indicate a serious underlying condition.
    • Avoid using other antihistamine-containing products, including topical antihistamines and cough and cold medicines.
    • Elderly patients with confusion should avoid using this medication, as they are more prone to adverse effects.
    • Do not use Cosome E continuously for more than 2 weeks without consulting a doctor.
    • Cosome E may enhance the effects of alcohol; therefore, alcohol should be avoided.
  • Drug Interactions

    Drug interactions with this Aminophylline, ammonium chloride, diphenhydramine combinaton include:

    • Caffeine (additive effects on theophylline levels)
    • Rifampin (decreases theophylline levels)
    • Carbamazepine (decreases theophylline levels)
    • Erythromycin (increases theophylline levels)
    • Ketoconazole (increases theophylline levels)
    • Warfarin (increased risk of bleeding)
    • Alcohol (may enhance sedative effects of diphenhydramine)
    • Other CNS depressants (additive sedation)
    • Antacids (may alter absorption of theophylline)
    • Antihypertensives (may have additive hypotensive effects)
  • Food Interactions

    The food interactions that should be considered while using Aminophylline, Ammonium Chloride, and Diphenhydramine are:

    • Alcohol can increase the sedative effects of many antihistamines and cough suppressants, leading to increased drowsiness and impaired motor function. It’s generally advised to avoid alcohol while taking Hydryllin.
    • Grapefruit juice typically doesn’t affect the common ingredients in cough syrups, but it’s always wise to check with your healthcare provider.
    • Caffeine doesn’t usually interact with Hydryllin syrup, but if the syrup causes drowsiness, caffeine might counteract this effect. Be mindful of your overall caffeine intake.
    • High-fat foods can sometimes affect the absorption rate of medications. If you notice any issues, consider taking the syrup with a light meal or as directed by your healthcare provider.
    • Spicy or acidic foods might irritate your throat further if you’re taking Hydryllin for a cough. If you’re experiencing throat irritation, it might be best to avoid such foods.
  • Storage/Disposal

    Cosome-E 120ml Cough Syrup should be stored properly to maintain its effectiveness.
