Prostaglandins are involved in pain, inflammation, and blood clotting. Aspirin stops the production of prostaglandins, which reduces pain and swelling in the body.Aspirin also prevents platelets (cells involved in blood clotting) from clumping together by blocking prostaglandin production. In the event of an overdose or early poisoning with aspirin, symptoms may include burning in the mouth, lethargy, nausea, vomiting, tinnitus, sweating, thirst, tachycardia, or dizziness. A moderate overdose presents with additional symptoms such as tachypnea, hyperpyrexia, dehydration, loss of coordination, restlessness, and mental confusion. In severe cases, symptoms escalate to include hypotension, hallucinations, stupor, hypoglycemia, convulsions, cerebral edema, oliguria, renal failure, cardiovascular failure, coma, hemorrhage, metabolic acidosis, and/or respiratory failure. Emergency management involves immediate hospital transfer, where cardiovascular and respiratory support must be maintained. Treatment includes gastric lavage and the administration of activated charcoal, with close monitoring and correction of acid-base imbalances if necessary. Alkaline diuresis should be considered when plasma salicylate concentrations exceed 500 mg/L in adults or 300 mg/L in children. Hemodialysis may be required in severe cases where salicylate levels are above 800 mg/L in adults or 700 mg/L in children, or if there are significant systemic symptoms such as severe metabolic acidosis, acute renal failure, pulmonary edema, or central nervous system issues like drowsiness, agitation, coma, or convulsions. Fluid losses should be replaced with a hypotonic solution, such as half saline, supplemented with glucose. Symptomatic treatment should be provided as needed. Fatal doses of aspirin range from 10 to 30 grams, though there have been cases where up to 130 grams were ingested without a fatal outcome. For suspected overdose, contact the regional Poison Control Centre immediately.
