    Famotidine is a drug that acts as a competitive histamine H-receptor antagonist (H2RA) it binds to the H-receptors that are located on the basolateral membrane of the parietal cell in the stomach; in this way it effectively blocks histamine actions. Its pharmacologic actions lead to the inhibition of gastric secretion by suppressing acid concentration and volume of gastric secretion. Famotidine inhibits both basal and nocturnal gastric acid secretion; it also decreases gastric volume, acidity, and secretion that are stimulated by food, caffeine, insulin, and pentagastrin(synthetic polypeptide that acts like gastrin).

    • Duodenal Ulcer

  • Dosage

    Dose must be taken according to the doctor’s advice only; doses given below are suggested as supportive literature for better understanding in some areas of requirements. Adult dose: For Duodenal Ulcer Acute treatment; 20mg orally every 12 hours or 40mg orally at bedtime for 8 weeks. Maintenance dose is; 20mg orally at bedtime. For Reduction of recurrence risk; 20mg orally every Day for 1 year or as clinically suggested. For Benign Gastric Ulcer; 40mg orally at bedtime up to 8 weeks. For Gastroesophageal Reflux Disease (Nonerosive); 20mg every 12hours, up to 6 weeks, (Erosive diagnosed by endoscopy); 20-40mg orally every 12hours for up to 12 weeks. For Hypersecretory Conditions; 20mg orally every 6 hours; may increase up to 160mg every 6 hours. For Heartburn; 10-20mg every 12 hours; may take 15-60 minutes before eating foods that could cause heartburn. Child dose: For Peptic Ulcer: 1-17 years, 0.5mg/kg orally at bedtime; may increase up to 1mg/kg once daily at bedtime or 0.5mg/kg twice daily for up to 8 weeks; do to exceed up to 40mg per day. For Gastroesophageal Reflux Disease; In children <3 months: 0.5mg/kg orally once daily; may increase up to 1mg/kg once daily for up to 8 weeks. In children of 3-12 months, 0.5mg/kg orally every 12 hours; may increase up to 1mg/kg twice daily; for up to 8 weeks; do not exceed up to 40mg per day. In children of age 1-17 years with or without esophagitis and ulceration; 0.5mg/kg twice daily; for 6-12 weeks; do not exceed up to 40mg twice daily. For Heartburn; >12 years, 10-20mg every 12 hours; may take 15-60 minutes before eating foods that could cause heartburn.

  • In case of Overdose

    Famotidine overdose may cause; abnormal heartbeat- including rapid or slow heartbeat, agitation, drowsiness, confusion, diarrhea, difficulty breathing, hallucinations, low blood pressure and flushing. Consult your doctor in case of overdose.

    If you miss any dose then take the dose as soon as you remember, but if it's the time for the next dose then do not double the dose.

    Do not use in case of hypersensitivity to Famotidine or any H2 receptor blocker drug. Do not use it if you are facing trouble or pain when swallowing food, vomiting with blood, or bloody or black stools; kidney impairment; coadministration with other acid reducers.

    Famotidine can cross placenta so it should be used with caution during pregnancy. Among Histamine 2 anatagonists famotidine is the drug that is present in breast milk in lowest concentration, however caution is required while prescribing this to lactating mothers.

    The drug interactions with famotidine include:

    • Antacids – Taking antacids with famotidine can reduce its absorption and effectiveness. It is advisable to separate the administration of antacids and famotidine by at least 30 minutes.
    • Anticoagulants – Famotidine may increase the risk of bleeding when used with anticoagulants such as warfarin by altering the metabolism and clearance of the anticoagulant.
    • Ketoconazole – Famotidine can decrease the absorption of ketoconazole due to reduced gastric acidity, potentially decreasing the effectiveness of ketoconazole.
    • Digoxin – The absorption of digoxin can be reduced by famotidine, potentially affecting its efficacy.
    • Theophylline – Famotidine may alter the metabolism of theophylline, potentially affecting its clearance and increasing the risk of side effects.
    • Methotrexate – Famotidine may increase methotrexate levels, leading to an increased risk of methotrexate toxicity.
    • Warfarin – Concomitant use with famotidine can lead to altered warfarin levels and increased bleeding risk.
    • Procainamide – Famotidine can affect the metabolism of procainamide, requiring dose adjustments and careful monitoring.
    • Clopidogrel – Famotidine may potentially interact with clopidogrel by altering its absorption, but clinical significance is generally minimal.
    • Rifampin – Rifampin may reduce the effectiveness of famotidine by increasing its metabolism.

    Long term use ≥2 years may lead to Vitamin B12 deficiency, due to malabsorption of dietary vitamin B12. Avoid the use of caffeine (coffee) along with use of this drug.
