  • Description

Rifaximin is a nonsystemic antibiotic, meaning it targets the gastrointestinal tract specifically. It inhibits RNA polymerase, which results in the inhibition of bacterial protein synthesis and killing of the microorganisms. This action is what makes it a broad-spectrum antibiotic, active against various gram-positive and gram-negative bacteria, including anaerobic bacteria. When traveler's diarrhea occurs, rifaximin is administered to inhibit the mobilization of bile acids from the liver, minimizing the growth of bacteria in the gastrointestinal tract known to cause this condition. It is also believed to have a direct antimicrobial effect on the bacteria itself. In terms of hepatic encephalopathy, rifaximin can help reduce the risk of breakthrough infections by reducing the luminal bacterial load, altering the microbiome, and decreasing production of toxic metabolites by intestinal bacteria. By reducing bacterial counts and altering the intestinal microbiome, rifaximin's antibiotic properties position it as a beneficial treatment for traveler's diarrhea and hepatic encephalopathy.

    • For travelers' diarrhea in adults- 200 mg initially, then 100 mg every 8 hours for 2 days.
    • For hepatic encephalopathy in adults- 800 mg daily in 2 divided doses.
    • For irritable bowel syndrome in adults- 550 mg initially, then 200 mg taken twice a day.
  • In case of Overdose

In the event of a rifaximin overdose, it is crucial to seek immediate medical attention from a professional healthcare provider or emergency services. Depending on the severity of the overdose, one may require supportive care, which could include gastrointestinal decontamination through the use of activated charcoal or pumping of the stomach to rid the body of the excess medication. Should the overdose present more seriously, hospitalization may be necessary for closer monitoring, intravenous (IV) fluids, or further interventions. As rifaximin use is known to disrupt the intestinal microbiome, probiotics or other microbiome-targeting therapies may also be recommended as part of the treatment plan in some cases. Regardless of the perceived severity of the overdose, it is essential to seek emergency medical attention to ensure proper care and treatment individualized to the patient.

  • Missed Dose

If you miss a dose of rifaximin, it is important to take prompt action. You should take the missed dose as soon as possible, but if it is already time for your next scheduled dose, simply skip the missed dose and resume your regular dosage schedule. It is critical to avoid overdosing by not taking twice the recommended dosage to make up for a missed one. If you are uncertain about how to proceed or have other concerns, it is always advisable to consult with your healthcare provider. They can provide tailored guidance based on your specific condition, medical history, and any potential interactions with other medications you may be taking. Ultimately, adhering to the prescribed medication plan and following your healthcare provider's instructions precisely is essential for maximizing the effectiveness of your treatment and promoting optimal long-term health outcomes.
