Product Information
Irbesartan is a selective competitive angiotensin II receptor type 1 (AT1) antagonist. Angiotensin II is a vasoconstrictor that promotes renal sodium retention and potassium excretion, increases blood pressure, causes peripheral arterial vasoconstriction, cardiac ventricular hypertrophy, and myocardial contractility. By blocking the AT1 receptor, Irbesartan prevents increased blood pressure and cardiac contraction. Irbesartan's efficacy is based on both peripheral and renal (nephron-protective) actions. Hydrochlorothiazide is a thiazide diuretic that inhibits the sodium-chloride symporter in the distal renal tubule. By inhibiting reabsorbtion of sodium and chloride ions, hydrochlorothiazide promotes natriuresis and distal renal tubular water conservation, resulting in increased urine volume and decreased blood pressure. It also inhibits calcium reabsorbtion leading to increased excretion and decreased plasma levels. The decreased blood pressure further reduces sympathetic neural stimulation of the renin-angiotensin-aldosterone system (RAAS), which exacerbates hypertension. Hydrochlorothiazide has a relatively low potassium-sparing effect, increasing the risk of potassium deficiency. Together, Irbesartan and Hydrochlorothiazide work in conjunction to help treat hypertension.
