    Ivabradine is a novel selective cardiac medication that works by inhibiting the cardiac myocardial hyperpolarization current (If). This current inhibition decreases the efflux of sodium ions from the cardiac sarcolemmal sodium-potassium ATPase pump, resulting in decreased contractility and a decreased heart rate. This mechanism of action differs from that of beta-blockers, and it avoids some of the side effects associated with beta-blockers, such as increased airway resistance and arterial blood pressure.

    Ivabradine is utilized for the following therapeutic indications:

    • Management of chronic stable angina pectoris in patients with normal sinus rhythm
    • Treatment of heart failure with reduced ejection fraction
    • Used to reduce the risk of hospitalization for worsening heart failure
    • Effective in patients who cannot tolerate beta-blockers or in addition to beta-blocker therapy
  • Dosage

    Tailoring Treatment for Optimal Health, Prescribed Medication Dosage is:

    • Chronic stable angina- Initial treatment- Start at 5 mg once daily in the morning, increasing to 7.5 mg once daily if tolerated. Maximum dose- 7.5 mg once daily
    • Acute coronary syndrome (ACS) undergoing PTCA- Can be started intravenely at a dose of 0.15 mg/kg body weight over 10 minutes, followed by a 5 mg tablet orally, before discharge from the hospital. Maintenance dose should be adjusted to maintain a heart rate of 50 to 60 beats per minute. Avoid exceeding an IV dose of 1.4 mg
    • Hypertension- Ivabradine is not recommended as first-line therapy, but if used, the starting dose is 7.5 mg once daily, with titration up to a maximum of 15 mg once daily
    • Severe bradycardia or symptomatic hypotension- If the heart rate drops below 50 beats per minute, treatment should be interrupted and, if necessary, treatment with a beta-agonist or other available means should be considered to increase the heart rate
    • Impaired renal function (CrCl <70 ml/min)- No dosage adjustment is required in patients with mild to moderate renal impairment
    • In patients with severe renal impairment (CrCl <30 ml/min) or end-stage renal disease (ESRD) who are undergoing dialysis, the starting dose should be reduced to 2.5 mg once daily
  • In case of Overdose

    An Ivabradine overdose is a serious and potentially life-threatening situation that requires immediate medical attention. The first response to an overdose includes efforts to empty the patient's stomach using induced vomiting or lavage, as well as the administration of charcoal to prevent further absorption of the drug. Laxatives are also given to promote elimination. During the treatment process, it is imperative that doctors closely monitor the patient's vital signs and oxygen saturation, as well as their heart rate with comprehensive cardiovascular monitoring in place. To treat symptoms of bradycardia that may arise as a result of the overdose, beta-blockers may be administered. In severe cases, intensive care and respiratory support may be needed to address complications. Rapid and skilled medical intervention can make a big difference in the outcome, significantly improving the patient's chances of a positive result.
