    Lactulose, a synthetic disaccharide, primarily acts as an osmotic laxative. It is not absorbed in the small intestine and reaches the colon unchanged, where it is fermented by the resident bacteria. This fermentation process results in the production of organic acids, primarily lactic acid and acetic acid, along with gases such as carbon dioxide and hydrogen. These organic acids and gases increase the osmotic pressure within the colon, causing water retention in the stool, which softens it and promotes bowel movements.
