• Mixed dyslipidaemia, Primary hypercholesterolaemia - Usual initial dose - 10 mg or 20 mg once daily - patients requiring >45% reduction in LDL-C may start at 40 mg once daily. Usual range - 10-80 mg once daily. Max - 80 mg once daily.
• Homozygous familial hypercholesterolaemia - 10-80 mg daily as an adjunct to other lipid-lowering treatments (e.g., LDL apheresis).
• Prophylaxis of cardiovascular events in high-risk patients - Initially, 10 mg daily. Higher doses may be required to meet LDL-C levels according to current guidelines.

In the event of an overdose with LIPITOR (atorvastatin), there is no specific antidote available. Treatment should be symptomatic and supportive, focusing on managing any symptoms that arise and providing supportive care as needed. Given that atorvastatin is extensively bound to plasma proteins, hemodialysis is not anticipated to significantly enhance its clearance from the body. Therefore, the primary approach is to monitor the patient closely and provide appropriate supportive measures.

Take the missed dose as soon as you remember. If it’s almost time for the next dose, skip the missed dose. Do not double up.

Take tablet daily, preferably at the same time each day. Swallow whole with a glass of water.

Do not use atorvastatin:

• If you are hypersensitive to any component of this medication.
• If you have active liver disease or unexplained persistent elevations of serum transaminases.
• If you are pregnant, as safety in pregnancy has not been established.
• If you are breastfeeding, due to the potential for serious adverse effects in nursing infants.

• Avoid concurrent use with clarithromycin, itraconazole, fosamprenavir, ritonavir (especially when used with darunavir, fosamprenavir, or saquinavir), elbasvir/grazoprevir combination, letermovir, boceprevir, or nelfinavir.
• Consider genetic testing if you have known SLCO1B1 gene polymorphism.
• Use with caution in individuals with renal impairment, as well as in children and the elderly.

Drug interactions with atorvastatin include:

• CYP3A4 inhibitors (e.g., ketoconazole, erythromycin) (may increase atorvastatin levels, risk of statin-related side effects)

• Grapefruit juice (may increase atorvastatin levels)
• Immunosuppressants (e.g., cyclosporine) (increased risk of statin side effects)
