  • Description

Adenosine, when administered as eye drops, primarily acts on the A1 and A2A adenosine receptors in the ocular tissues. Its mechanism of action involves the direct activation of these receptors, leading to a decrease in aqueous humor production. Adenosine stimulates A1 receptors on ciliary epithelial cells, inhibiting the activity of adenylate cyclase, which in turn reduces the formation of cAMP. This reduction in cAMP decreases the secretion of fluid into the eye, thereby lowering intraocular pressure. Additionally, adenosine may enhance uveoscleral outflow, further contributing to its overall effect on reducing intraocular pressure. The selective targeting of adenosine receptors makes it an effective agent for managing certain ocular conditions, providing a localized therapeutic response.

  • In case of Overdose

In the case of an overdose of Adenosine Drops, it is crucial to act promptly. Symptoms may include severe bradycardia, hypotension, and even cardiac arrest. Immediate medical intervention is necessary, including supportive care and monitoring of vital signs. Treatment may involve managing symptoms and ensuring adequate oxygenation and ventilation. Prompt recognition and medical care are essential to mitigate the risks associated with Adenosine Drop overdoses.

  • Missed Dose

In the case of a missed dose of Adenosine Drops, it is important to follow the instructions provided by your healthcare professional. If you remember within a short time of the missed dose, administer the medication as soon as possible. However, if it is almost time for the next scheduled dose, skip the missed dose and continue with your regular dosing schedule. Do not give a double dose to make up for a missed one, as this may increase the risk of side effects. Consult your healthcare provider if you have any concerns or questions regarding missed doses.

  • How To Use

To administer the Adenosine Drop, gently tilt your head back, instill the prescribed number of drops into the conjunctival sac of the affected eye(s), and then close your eyes for at least 1-2 minutes to allow for proper absorption.

  • When Not to Use

Following are the conditions or situations when Adenosine drops should not be used:

    • Should not be used in patients with known severe hypersensitivity or allergy to Adenosine or any of its components.
    • Contraindicated in patients with certain heart conditions, such as sick sinus syndrome, second• or third-degree atrioventricular (AV) block, or severe hypotension.
    • Not recommended for patients with severe asthma or acute myocardial infarction.
    • Should not be used in patients with untreated angina or severe aortic valve stenosis.
    • Contraindicated in patients with severe lung disease or primary pulmonary hypertension.

  • Precautions & Warnings

Following are the precautions and warnings for Adenosine Drop:

    • Do not use if you have a known allergy to Adenosine or any excipients in the formulation.
    • It should be administered with caution in patients with certain heart conditions, such as sick sinus syndrome, atrial flutter, or atrial fibrillation.
    • Adenosine requires careful monitoring during administration due to its potential to cause transient cardiac side effects.
    • Not recommended for use in patients with severe low blood pressure or unstable angoric chest pain.
    • Elderly patients may require lower doses as they can be more sensitive to the drug's effects.
    • This medication is not recommended for use during pregnancy or breastfeeding unless the potential benefits outweigh the risks.
    • Adenosine should be used with caution in patients with impaired liver function as it is metabolized in the liver.
  • Drug Interactions

Following are the drug interactions for Adenosine Drop:

    • Dipyridamole, a vasodilator, can enhance the actions of Adenosine, leading to prolonged heart block.
    • Anticoagulants like Warfarin may have their effects potentiated, increasing the risk of bleeding.
    • Theophylline, a bronchodilator, can reduce the effectiveness of Adenosine.
    • Caffeine, a stimulant, may counteract the effects of Adenosine, requiring higher doses for treatment.
    • Carbamazepine, an anticonvulsant, can increase the metabolism of Adenosine, reducing its effectiveness.
    • When combined with Digoxin, Adenosine may cause cardiac arrhythmias.
    • Beta-blockers, such as Propranolol, can decrease the clearance of Adenosine, resulting in prolonged heart block.
  • Food Interactions

Following are the food interactions to consider with Adenosine Drops:

    • Avoid consuming large amounts of caffeine-rich foods and beverages like coffee, tea, energy drinks, and chocolate, as caffeine may counteract the effects of Adenosine and lead to reduced effectiveness.
    • Grapefruit and grapefruit juice should be avoided as they can interfere with the body's metabolism, potentially increasing the concentration of Adenosine in the blood and enhancing its effects.
    • Limit the consumption of high-purine foods such as organ meats, anchovies, sardines, and game meats. Purines can be broken down into uric acid, and Adenosine may affect its metabolism, potentially leading to increased uric acid levels.
    • Alcohol may have additive effects when combined with Adenosine, increasing the risk of side effects like dizziness, lightheadedness, and hypotension. It's advisable to limit or avoid alcohol consumption during treatment.
    • Avoid excessive consumption of foods high in sugar, especially refined sugars, as they can impact blood glucose levels, which may be relevant if Adenosine is used in the context of cardiac or circulatory conditions.
    • Maintain a balanced diet rich in fruits, vegetables, whole grains, and lean proteins to support overall health and cardiovascular function.
    • It's generally recommended to follow a healthy diet and stay well-hydrated to ensure optimal drug efficacy and overall well-being during Adenosine therapy.
  • Storage/Disposal

Adenosine Drop should be stored in a cool and dry place, preferably at room temperature, ensuring it is kept away from direct exposure to sunlight, heat, or moisture. It is crucial to store the medication in its original container, tightly sealed, to maintain its integrity and prevent contamination. Do not transfer the medication to another container as this may affect its quality. When disposing of Adenosine Drop, do not flush it down the toilet or pour it into a drain. Instead, follow local guidelines for medication disposal or consult your pharmacist for environmentally safe disposal methods. Always keep the medication out of the sight and reach of children to prevent accidental ingestion.

  • Quick Tips, if any

    • Follow the prescribed dosage and administration instructions provided by your healthcare professional.
    • Ensure the dropper is clean and sterilized before each use to prevent contamination.
    • Administer the drops directly into the affected eye(s) as directed, avoiding contact with other surfaces.
    • Avoid touching the dropper tip to your eye or any surface to maintain sterility.
    • If using other eye medications, wait at least 5 minutes between different eye drops to prevent dilution.
    • Store the drops as directed on the packaging, usually at room temperature and away from direct sunlight.
