Dosulpin inhibits the reuptake of norepinephrine and serotonin by blocking their respective transporters in the presynaptic neurons. This leads to an increased concentration of these neurotransmitters in the synaptic cleft, enhancing mood and alleviating symptoms of depression. Additionally, dusolpin has anticholinergic properties due to its antagonism of muscarinic receptors, which can contribute to its sedative effects. It also exhibits weak antagonism at histamine H1 receptors, further contributing to its sedative and anxiolytic effects.

Prothiaden dosage recommendations:

• Adult Dosage

As a starting dose, 3 capsules a day. This may be taken as one capsule three times a day or 3 capsules as a single dose each evening, preferably a couple of hours before bedtime. Normally, not more than 9 capsules (equivalent to 225 mg dosulepin) should be taken each day.

Symptoms of overdosage may include dryness of the mouth, excitement, ataxia, drowsiness, loss of consciousness, muscle twitching, convulsions, widely dilated pupils, hyperreflexia, sinus tachycardia, cardiac arrhythmias, hypotension, hypothermia, depression of respiration, visual hallucinations, delirium, urinary retention, paralytic ileus, and respiratory or metabolic alkalosis. Treatment should consist of gastric lavage. When the patient is unconscious or the cough reflex depressed, the lungs should be protected by a cuffed endotracheal tube. Repeated gastric/intestinal aspiration or repeated administration of activated charcoal may remove the drug and metabolites excreted into the gut via the bile. Continuous ECG monitoring is advisable. Abnormalities of cardiac rhythm and epileptic convulsions may occur and should be treated accordingly. Forced diuresis is not recommended. Bedrest is advisable, even after recovery.

If you miss a dose, take it as soon as you remember. If it is close to the next dose, skip the missed dose. Do not double up.

Swallow the tablet whole with water, preferably at the same time each day. Follow the dosage instructions from your healthcare provider.

Dosulpin should not be used in the following situations:

• Hypersensitivity to the active substance or any excipients listed
• Allergy to peanut or soya (contains soya lecithin)
• Recent myocardial infarction
• Heart block or other cardiac arrhythmias
• Mania
• Severe liver disease
• Narrow-angle glaucoma or other causes of increased intraocular pressure

• Weight gain

Patients should be aware of the following precautions and warnings:

• Individuals with a history of bipolar disorder or psychosis should use this medication with caution.
• It is not recommended for patients with severe cardiovascular disease, including heart failure, conduction disorders, or cardiac arrhythmias, and those with risk factors for QTc prolongation or torsades de pointes (TdP), such as congenital long QT syndrome.
• Those with a significant risk of suicide should be monitored closely.
• Caution is advised for patients with glaucoma, urinary retention, or prostatic hypertrophy.
• Avoid abrupt withdrawal of the medication to prevent potential complications.
• Use with care in elderly patients.
• Pregnant or breastfeeding women should consult their healthcare provider before use.

Drug interactions with dosulepin hydrochloride include:

• Other antidepressants (e.g., selective serotonin reuptake inhibitors (SSRIs)) (may increase the risk of serotonin syndrome- use with caution)
• Monoamine oxidase inhibitors (MAOIs) (e.g., phenelzine) (may cause severe hypertension- avoid concomitant use)
• CNS depressants (e.g., alcohol, benzodiazepines) (may enhance sedative effects- monitor for increased sedation)
• Anticholinergic drugs (e.g., atropine) (may increase anticholinergic side effects such as dry mouth and constipation)
• CYP450 inhibitors (e.g., ketoconazole) (may increase dosulepin levels- monitor for increased side effects)
• CYP450 inducers (e.g., rifampin) (may decrease dosulepin levels- monitor for decreased efficacy)
• Antihypertensive agents (e.g., beta-blockers) (may enhance hypotensive effects- monitor blood pressure)
• Warfarin (may increase bleeding risk- monitor INR closely)
• Antiepileptic drugs (e.g., phenytoin) (may decrease dosulepin levels- monitor for reduced efficacy)

Dosulepin Hydrochloride should be stored properly to maintain its effectiveness.
