Paracetamol, also known as acetaminophen, is a commonly used analgesic and antipyretic medication. Paracetamol works by inhibiting the synthesis of prostaglandins in the brain, which are chemicals involved in pain and fever. Paracetamol is often used as a first-line treatment due to its efficacy and relatively mild side effect profile. Ibuprofen is a nonsteroidal anti-inflammatory drug (NSAID) that works by inhibiting the activity of cyclooxygenase (COX) enzymes, specifically COX-1 and COX-2. These enzymes are crucial in the biosynthesis of prostaglandins, which are chemicals that mediate inflammation, pain, and fever.

Ibuprofen + Paracetamol 200 mg/500 mg dosage recommendations are:

• General Use (for pain and fever relief) - Take 1 tablet with water and food, up to 3 times a day. If one tablet does not control symptoms, a maximum of 2 tablets may be taken up to three times a day. Do not exceed 6 tablets in any 24-hour period (equivalent to 3000 mg Paracetamol and 1200 mg Ibuprofen).

If you think that you have used too much paracetamol + Ibuprofen combination, you may need urgent medical attention.

Take the missed dose as soon as you remember. If it is almost time for your next dose, skip the missed dose. Do not take double doses.

Take tablet daily with water, preferably at the same time each day.

Do not use Paracetamol + Ibuprofen combination if you have:

• you are allergic to ibuprofen, paracetamol or any of the ingredients listed at the end of this leaflet.
• you are allergic to any other medicine for pain relief
• Liver or kidney disease
• Heart problems
• Asthma
• A history of stomach or duodenal ulcers, gastric bleeding, or gastrointestinal diseases
• Recently vomited blood or material resembling coffee grounds
• Recently bled from the rectum or had black sticky bowel motions
• The expiry date has passed
• The packaging is torn or shows signs of tampering
• Using other products containing paracetamol, ibuprofen, or other NSAIDs

The side effects of Paracetamol and Ibuprofen combination are as follows:

• Gastrointestinal discomfort
• Headache
• Dizziness
• Allergic reactions
• Fatigue

Here are some precautions and warnings that patients should keep in mind:

• Patients with existing or a history of bronchial asthma or allergic diseases should be cautious.
• Individuals with a history of hypertension or mild to moderate congestive heart failure (CHF) require careful monitoring.
• Those with mixed connective tissue disorders or systemic lupus erythematosus (SLE) should consult their healthcare provider.
• Patients with non-cirrhotic alcoholic liver disease should take special care.
• Caution is advised for individuals with G6PD deficiency, chronic malnutrition, anorexia, bulimia, cachexia, dehydration, or hypovolemia (especially with IV administration).
• Elderly patients should be closely monitored for adverse effects.
• Patients with renal and hepatic impairment require special attention.
• Extra care is necessary during the 1st and 2nd trimesters of pregnancy and for lactating women (especially with oral administration).

Drug interactions with ibuprofen and paracetamol include:

• Anticoagulants (e.g., warfarin) (ibuprofen may enhance the effect of anticoagulants)
• Alcohol (chronic use may increase the risk of liver damage from paracetamol and gastrointestinal issues from ibuprofen)
• Other NSAIDs (e.g., naproxen) (may increase the risk of gastrointestinal bleeding)
• Antihypertensives (e.g., ACE inhibitors) (ibuprofen may reduce the effectiveness of antihypertensives)
• Corticosteroids (e.g., prednisone) (may increase the risk of gastrointestinal adverse effects)

Store at room temperature in a tightly closed container. Dispose of unused or expired medication by returning it to a pharmacy for safe disposal.
