Selanz (30mg) 14 Capsules

Lansoprazole targets the proton pumps located in the parietal cells of the stomach lining. These proton pumps are responsible for secreting hydrogen ions (protons) into the stomach, which combine with chloride ions to form hydrochloric acid.Lansoprazole works by binding to and inhibiting these proton pumps, thereby blocking the final step of acid production in the stomach.

Proton Pump Inhibitor (PPI)

This medication is used to:

• Treat Acid Reflux
• Manage Gastric Issues
• Reduce Heartburn
• Alleviate Indigestion

Lansoprazole dosage recommendations are:

• Gastroesophageal reflux disease (GERD) - 15-30 mg once daily, may increase to 60 mg once daily, Children (aged 1-11 years) - 15-30 mg once daily
• Peptic ulcer disease - 15-30 mg once daily, may increase to 60 mg once daily, Children (aged 1-11 years) - 15-30 mg once daily
• Zollinger-Ellison syndrome - 60 mg once daily, may increase to 120 mg twice daily

Immediately telephone your doctor or the Poisons Information Centre for advice or go to Accident and Emergency at the nearest hospital, if you think that you or anyone else may have taken too much of this medicine. Do this even if there are no signs of discomfort or poisoning.

If it is almost time for your next dose, skip the missed dose and continue with your regular dosing schedule. However, if it is not close to the next dose, take the missed dose as soon as you remember and then resume your usual dosing routine. Avoid taking a double dose to compensate for the missed one, as this could increase the risk of unwanted side effects. If you are uncertain about what to do or need advice, consult your doctor or pharmacist. The capsule should be swallowed whole with plenty of water and should not be crushed or chewed. If swallowing the capsule is difficult, you may open it and take the granules as follows:

Do not take Lansoprazole if you have an allergy to:

• Lansoprazole or other proton pump inhibitors.
• Any of the ingredients listed at the end of this leaflet.
• Do not take this medicine if you have severe liver disease.
• Do not take this medicine if you are already taking atazanavir, a medicine used to treat HIV.

• Diarrhea
• Headache
• Xerostomia
• Fatigue
• Arthralgia
• Insomnia
• Skin rash
• Weakness

Before using this medication, it is important to be aware of the following precautions and warnings:

• Caution is advised for patients with reduced body stores or risk factors for reduced vitamin B12 absorption, and those at risk of osteoporosis.
• The medication may mask symptoms of gastric malignancy.
• Consider the patient’s CYP2C19 metabolic status, including ultrarapid, rapid, normal, intermediate, and poor metabolizers.
• Use with care in individuals with renal and moderate to severe hepatic impairment.
• Special consideration is required when administering this medication to children and the elderly.
• Consult a healthcare professional before using the medication during pregnancy and lactation to ensure safety.

Drug interactions with lansoprazole include:

• CYP2C19 substrates (e.g., clopidogrel) (may reduce activation of clopidogrel- monitor for reduced efficacy)
• CYP3A4 substrates (e.g., midazolam, tacrolimus) (may increase levels of these drugs- monitor for side effects and adjust doses as necessary)
• Antacids (e.g., magnesium hydroxide) (may reduce lansoprazole absorption- take lansoprazole at least 30 minutes before antacids)
• H2-receptor antagonists (e.g., ranitidine) (may reduce lansoprazole efficacy- monitor for reduced effectiveness)
• Warfarin (may enhance anticoagulant effects- monitor INR closely)
• Methotrexate (may increase methotrexate levels- monitor for methotrexate toxicity)
• Azole antifungals (e.g., ketoconazole) (may increase levels of these drugs- monitor for side effects)
• Rifampin (may decrease lansoprazole levels- monitor for reduced efficacy)

Store at room temperature, away from moisture and heat. Dispose of unused or expired medication by returning it to a pharmacy for safe disposal.
