    Ivabradine is a novel selective cardiac medication that works by inhibiting the cardiac myocardial hyperpolarization current (If). This current inhibition decreases the efflux of sodium ions from the cardiac sarcolemmal sodium-potassium ATPase pump, resulting in decreased contractility and a decreased heart rate. This mechanism of action differs from that of beta-blockers, and it avoids some of the side effects associated with beta-blockers, such as increased airway resistance and arterial blood pressure.

    • Chronic stable angina- Initial treatment- Start at 5 mg once daily in the morning, increasing to 7.5 mg once daily if tolerated. Maximum dose- 7.5 mg once daily
    • Acute coronary syndrome (ACS) undergoing PTCA- Can be started intravenely at a dose of 0.15 mg/kg body weight over 10 minutes, followed by a 5 mg tablet orally, before discharge from the hospital. Maintenance dose should be adjusted to maintain a heart rate of 50 to 60 beats per minute. Avoid exceeding an IV dose of 1.4 mg
    • Hypertension- Ivabradine is not recommended as first-line therapy, but if used, the starting dose is 7.5 mg once daily, with titration up to a maximum of 15 mg once daily
    • Severe bradycardia or symptomatic hypotension- If the heart rate drops below 50 beats per minute, treatment should be interrupted and, if necessary, treatment with a beta-agonist or other available means should be considered to increase the heart rate
    • Impaired renal function (CrCl <70 ml/min)- No dosage adjustment is required in patients with mild to moderate renal impairment
    • In patients with severe renal impairment (CrCl <30 ml/min) or end-stage renal disease (ESRD) who are undergoing dialysis, the starting dose should be reduced to 2.5 mg once daily
