Description
Solifenacin works by blocking muscarinic receptors in the bladder. These receptors, specifically the M3 subtype, are responsible for the contraction of the bladder muscle (detrusor muscle) during urination. By antagonizing these receptors, solifenacin reduces the activity of the detrusor muscle. This leads to less frequent bladder contractions, allowing the bladder to hold more urine and reducing the urgency and frequency of urination.

Uses
It is a muscarinic antagonist indicated for the treatment of overactive bladder with symptoms of urge urinary incontinence, urgency and urinary frequency. -
Dosage
Solifenacin dosage recommendations are:
- Overactive bladder - 5 mg once daily - may increase to 10 mg once daily if needed.
- Neurogenic detrusor overactivity - 1 mg/mL oral susp - 9-15 kg - Initially, 2 mg (Max - 4 mg) - >15-30 kg - Initially, 3 mg (Max - 5 mg) - >30-45 kg - Initially, 3 mg (Max - 6 mg) - >45-60 kg - Initially, 4 mg (Max - 8 mg) - >60 kg - Initially, 5 mg (Max - 10 mg). All doses are to be taken once daily - may titrate doses to the lowest effective dose after initiation.

In case of Overdose
In case of overdose, seek immediate medical attention. Symptoms of overdose may include severe dry mouth, blurred vision, or confusion. Contact poison control for guidance.

Missed Dose
If a dose is missed, take it as soon as you remember. If it's almost time for the next dose, skip the missed dose. Do not double up.

How To Use
Take with or without food as directed. Swallow the tablet whole with a glass of water.

When Not to Use
Do not use solifenacin in the following situations:
- If you are allergic to solifenacin or any of its ingredients
- If you have severe urinary retention
- If you have gastrointestinal obstruction
- If you have specific types of glaucoma

Side Effects
Dry mouth, GI upset, blurred vision. Reports of angioedema, anaphylaxis (discontinue), QT prolongation. -
Precautions & Warnings
Bladder outflow obstruction at risk o unnary retention, GI obstruction, risk of oesophagitis, reduced GI motility, severe renal or moderate hepatic impairment, autonomic neuropathy, hiatus hernia, oesophageal reflux. -
Drug Interactions
Drug interactions with solifenacin include:
- CYP3A4 inhibitors (e.g., ketoconazole, erythromycin) (may increase solifenacin levels)
- CYP3A4 inducers (e.g., rifampin) (may decrease solifenacin levels)
- Anticholinergic drugs (e.g., antihistamines, tricyclic antidepressants) (may have additive anticholinergic effects)
- Cholinesterase inhibitors (e.g., donepezil) (may reduce the effectiveness of cholinesterase inhibitors)
- Drugs that affect gastric emptying (e.g., metoclopramide) (may alter the effects of solifenacin)
