    Telmisartan is a selective antagonist of angiotensin II receptor type AT1, meaning it reduces the responses of organs to angiotensin II. Specifically, by blocking the binding of angiotensin II to the AT1 receptor, telmisartan inhibits the mediated vasoconstriction, secretion of aldosterone, and reabsorption of sodium tubes. All of this reduces the overall vascular resistance and blood pressure, and it does not affect the levels of angiotensin II metabolites nor does it affect the RAS-independent effects of angiotensin II. Amlodipine, on the other hand, is a calcium channel blocker, inhibiting calcium uptake in vascular smooth muscle cells, which reduces the tone of blood vessels, increasing the supply of blood and oxygen to the heart and peripheral organs. This action results in the desired decrease in blood pressure. By blocking the L-type calcium channels, amlodipine causes inhibition of calcium uptake in cardiac and vascular smooth muscle cells, leading to slowed calcium uptake and relaxation of the coronary and peripheral arteries, resulting in increased blood flow and delivery of oxygen to the heart, which is normally suppressed in patients with hypertension and ischemic heart disease.
