  • Description

Vancomycin, a glycopeptide antibiotic, exerts its bactericidal effect primarily through inhibition of cell wall synthesis in susceptible gram-positive bacteria. It specifically binds to the terminal D-alanyl-D-alanine residues of the cell wall precursor peptidoglycan, preventing its incorporation into the growing peptidoglycan chain. This binding disrupts the cross-linking process essential for cell wall integrity, leading to cell lysis and death. By interfering with this crucial step in bacterial cell wall formation, vancomycin displays potent activity against a range of gram-positive pathogens, including methicillin-resistant Staphylococcus aureus (MRSA) and certain streptococcal species.

  • Ingredients

    Vancomycin
  • Drug Class

    Glycopeptide antibiotic.

  • Dosage Form

    Injection

  • Uses

The following are the uses of Vancomycin injection:

    • Used for treating serious infections caused by methicillin-resistant Staphylococcus aureus (MRSA)
    • Used for life-threatening blood infections (bacteremia)
    • Used for bone and joint infections that require surgical intervention
    • Used for severe skin and soft tissue infections
    • Used for endocarditis, an infection of the heart's inner lining
    • Used for pneumonia caused by specific bacteria
    • Used as a last resort for infections resistant to other antibiotics
  • In case of Overdose

In the case of an overdose of Vancomycin injection, the severity of symptoms can vary depending on the amount ingested. Common signs of overdose may include hearing loss, dizziness, and kidney damage. Prompt medical intervention is necessary to manage these symptoms effectively. Treatment typically involves monitoring kidney function and providing supportive care. It is crucial to seek medical advice if an overdose is suspected to ensure appropriate management and prevent potential long-term complications.

  • Missed Dose

In the case of a missed dose of Vancomycin injection, it is important to administer the medication as soon as possible. If it is almost time for the next scheduled dose, skip the missed dose and continue with the regular dosing schedule. However, do not double the dose to make up for a missed one. It is crucial to maintain a consistent dosing interval to ensure the effectiveness of the treatment. If you frequently miss doses, inform your healthcare provider, as they may need to adjust your dosage or provide further guidance.

  • How To Use

To administer the Vancomycin injection, clean the injection site with an alcohol wipe, insert the needle at a 90-degree angle, and slowly inject the medication, then withdraw the needle and gently apply pressure with a sterile bandage if needed.

  • When Not to Use

Vancomycin injection should not be used in the following conditions:

    • Hypersensitivity or allergy to vancomycin or any other components of the injection.
    • Known or suspected renal impairment, unless closely monitored for renal function.
    • Pregnancy, unless the potential benefits justify the potential risks to the fetus. It should be used during pregnancy only if prescribed by a healthcare provider.
    • Breastfeeding, unless recommended by a healthcare provider, as vancomycin can pass into breast milk.
    • Individuals with a history of hearing problems or vestibular disorders, as vancomycin can cause ototoxicity (damage to the ear).
    • Concurrent use with other nephrotoxic or ototoxic drugs unless necessary, due to the increased risk of kidney or hearing damage.
    • In patients with severe intestinal inflammation (e.g., inflammatory bowel disease) without physician supervision.
  • Side Effects

The side effects of Vancomycin injection may include:

    • Redness, swelling, or pain at the injection site
    • Allergic reactions (skin rash, itching, hives)
    • Hearing loss or ringing in the ears (tinnitus)
    • Kidney damage (decreased urine output, swelling in legs or ankles)
    • Thrombophlebitis (inflammation of the veins)
    • Nausea and vomiting
    • Diarrhea
    • Superinfection (excessive growth of bacteria or fungi)
  • Precautions & Warnings

Here are the precautions and warnings for Vancomycin injection:

    • Allergic reactions are possible; patients with a known hypersensitivity to Vancomycin or related antibiotics should not use it.
    • Vancomycin should be used with caution in patients with impaired kidney function, as dosage adjustments may be necessary to prevent toxicity.
    • Prolonged or repeated use of Vancomycin may lead to bacterial resistance or superinfections; regular monitoring is essential.
    • Patients should be well-hydrated before and during treatment to reduce the risk of kidney toxicity.
    • Hearing and vestibular function should be monitored during treatment, especially in patients with pre-existing hearing disorders.
    • Vancomycin should be used with caution in neonates and infants, as they may be more susceptible to toxicity.
    • Pregnant and breastfeeding women should only use Vancomycin if the potential benefits outweigh the risks, and under close medical supervision.
    • Patients should be advised to complete the full course of treatment, even if they feel better, to ensure the infection is fully treated.
  • Drug Interactions

Following are the drug interactions for Vancomycin injection:

    • Concomitant use with nephrotoxic drugs (e.g., aminoglycosides, amphotericin B) can increase the risk of kidney damage.
    • The effectiveness of vancomycin may be reduced when used with neuromuscular blocking agents, leading to decreased neuromuscular blockade.
    • Vancomycin can interact with vecuronium, causing prolonged neuromuscular blockade and respiratory depression. Close monitoring is required.
    • Concurrent use with anesthetics may increase the risk of anaphylaxis and hypotension.
    • Anticonvulsants like phenytoin can decrease vancomycin levels, potentially reducing its efficacy.
    • Drugs affecting bone marrow function, such as chloramphenicol, may have additive effects on bone marrow suppression when used with vancomycin.
    • It may alter the effectiveness of vaccines, so live virus vaccines should be avoided during treatment.
  • Food Interactions

Following are the food interactions for Vancomycin injection:

    • Avoid consuming high-fat meals as they may interfere with the absorption of Vancomycin, potentially reducing its effectiveness.
    • Grapefruit and grapefruit juice should be avoided as they can alter the metabolism of Vancomycin, leading to increased drug levels in the body and potential toxicity.
    • Alcohol consumption should be minimized or avoided as it may increase the risk of gastrointestinal side effects and liver-related complications.
    • Avoid live or live-attenuated bacterial vaccines during Vancomycin therapy as the antibiotic may interfere with the vaccine's effectiveness.
    • It is advisable to maintain a well-balanced diet to support overall health and aid in recovery during the course of Vancomycin treatment.
    • Ensure adequate fluid intake to reduce the risk of kidney-related side effects associated with Vancomycin.
    • Follow any dietary recommendations provided by your healthcare provider, especially if you have pre-existing medical conditions or are on a specialized diet.
  • Storage/Disposal

Store Vancomycin injection at room temperature, away from direct sunlight and moisture, in its original packaging to maintain sterility. Protect it from freezing temperatures and extreme heat, as these can impact the medication's potency. Always check the expiration date before use and never use the medication if it has expired. Dispose of any unused or leftover Vancomycin injection according to local guidelines or through a medicine take-back program. Do not dispose of it in regular trash or pour it down the drain unless instructed to do so, as it can harm the environment. Keep the medication safely locked away from children and pets to prevent accidental exposure or ingestion.
