Vancomycin is a glycopeptide antibiotic that has a unique mode of action inhibiting the second stage of cell wall synthesis of susceptible bacteria. It prevents bacterial growth and infection from spreading.

500 mg IV every 6 hours OR 1 g IV every 12 hours.

If you miss a dose, make sure to take it as soon as possible the same day. If you don't remember missing the dose, don't double it. Consult your doctor on how to adjust the missing doses.
It is usually infused (injected slowly) over a period of at least 60 minutes once every 6 or 12 hours, but may be given every 8 hours in newborn babies. The length of your treatment depends on the type of infection you have.
Vancomycin hydrochloride is contraindicated in patients with known hypersensitivity to vancomycin, or any other excipients or in patients with previous anaphylaxis or major allergy with other glycopeptides.
