Voriconazole specifically inhibits the fungal enzyme lanosterol 14α-demethylase. By inhibiting lanosterol 14α-demethylase, voriconazole prevents the conversion of lanosterol to ergosterol (a crucial component of fungal cell membrane), leading to the accumulation of toxic sterol intermediates. The disruption of ergosterol synthesis results in a defective cell membrane, which compromises the fungal cell's structure and function, eventually leading to cell death.

Voriconazole dosage recommendations are:

• Candidaemia, Deep tissue candida infections, Invasive aspergillosis, Scedosporiosis, and Fusariosis - Adults and Elderly – Initial/loading dose – 400 mg every 12 hours for the first 24 hours if weight ≥40 kg, 200 mg every 12 hours if weight <40 kg – Maintenance dose – 200 mg twice daily for weight ≥40 kg, 100 mg twice daily for weight <40 kg – Dose may be increased to 300 mg twice daily based on response. Children (2-11 years) and Teenagers (12-14 years) weighing <50 kg – Initial/loading dose – 400 mg every 12 hours for the first 24 hours, followed by maintenance dose of 9 mg/kg twice daily (max 350 mg twice daily). Teenagers (12-14 years) weighing ≥50 kg and those older than 14 years – Initial/loading dose – 400 mg every 12 hours for the first 24 hours, followed by maintenance dose of 200 mg twice daily.

If you take more voriconazole than prescribed, or if someone else accidentally takes your tablets, seek medical advice immediately or go to the nearest hospital emergency department. Bring the box of voriconazole tablets with you. Overdosing on voriconazole may cause abnormal sensitivity to light, so it is important to get medical attention promptly.

It is important to take your voriconazole tablets regularly at the same time each day. If you forget to take one dose, take your next dose when it is due. Do not take a double dose to make up for a forgotten dose.

Take tablet as directed by your healthcare provider. Follow the dosage instructions carefully and complete the full course of treatment.

Voriconazole treatment should not be combined with the following medicines:

• Terfenadine (used for allergy)
• Astemizole (used for allergy)
• Cisapride (used for stomach problems)
• Pimozide (used for treating mental illness)
• Quinidine (used for irregular heartbeat)
• Ivabradine (used for symptoms of chronic heart failure)
• Rifampicin (used for treating tuberculosis)
• Efavirenz (used for treating HIV) in doses of 400 mg and above once daily
• Carbamazepine (used to treat seizures)
• Phenobarbital (used for severe insomnia and seizures)
• Ergot alkaloids (e.g., ergotamine, dihydroergotamine; used for migraine)
• Sirolimus (used in transplant patients)
• Ritonavir (used for treating HIV) in doses of 400 mg and more twice daily
• St. John’s Wort (herbal supplement)
• Naloxegol (used to treat constipation caused by opioids)
• Tolvaptan (used to treat hyponatremia or to slow kidney function decline in polycystic kidney disease)
• Lurasidone (used to treat depression)
• Venetoclax (used to treat chronic lymphocytic leukemia)

Avoid sunlamps and tanning beds. This medicine may cause adrenal gland problems. Check with your doctor right away if you or your child have darkening of the skin, diarrhea, dizziness, fainting, loss of appetite, mental depression, nausea, skin rash, unusual tiredness or weakness, or vomiting.

Drug interactions with voriconazole include:

• CYP3A4 substrates (e.g., simvastatin, tacrolimus) (may increase levels of these drugs)
